  • Observed soiled microwave.
  • Observed multiple packages of tofu stored at temperatures over 41 degrees F. CA: Tofu was discarded. Corrected On-Site.
  • Observed multiple potentially hazardous food products not date-marked whatsoever–these foods were held past 24 hours. CA: Food were discarded. Corrected On-Site.
  • Observed that there are no food temperature measuring devices in the facility.
  • Observed a pot of rice stored directly on top of another pot of rice. Rice was in direct contact with the bottom of the pan. CA: Foods were discarded. Keep all foods covered to prevent contamination. Corrected On-Site.
  • Observed wet wiping cloths stored on prep tables in between uses. CA: Please keep them stored in sanitizer bucket in between uses.
  • Observed multiple scoops stored directly inside of dry goods (flour, rice, etc) with handle in direct contact with said foods. Corrected On-Site
  • Observed plates in clean dish area not inverted to prevent contamination. CA: Invert all plates and utensils.
  • Observed cooler running at 56 degrees F. CA: Instructed PIC to get cooler fixed or replaced.
  • Observed soiled back splash, wire rack, cooler bottoms, prep unit coolers, sides of equipment, and walls. CA: Need a detail clean.
  • Observed that handwashing sink was not working. PIC stated that it has a leaking problem.
  • Observed multiple ceiling tiles missing.
  • Observed multiple light bulbs out under vent hood. CA: replace with new ones.
  • Legal notice #020959 was issued because facility has earned a ‘U’. Facility must correct all food-borne illness risk factors from inspection dated 11/10/16. Facility must obtain a passing follow-up inspection (with no public health interventions).
